A
family interested in or needing Project 12-Ways services
may contact the Project or the Department
of Children and Family Services (DCFS). DCFS professionals determine
a family's eligibility for Project 12-Ways services.
The family may then be referred to the Project. If appropriate services are
available the case will be accepted. When a case is accepted, Project 12-Ways
staff members interview the family typically at home. Others who may be interviewed
with parental permission include extended family members, doctors, teachers,
and school administrators. The Counselor and the family determine the needs
and identify goals they hope to reach with help from Project 12-Ways. An agreement
is signed committing both to an effort toward those goals. Two counselors usually
see a family (at the family's home) once or twice each week, but contact may
be more frequent if necessary. Services end when all goals have been met, or
at the discretion of the family, DCFS or Project 12-Ways.
